Insurance, In-Network:

Most health insurances will generally cover treatment for sleep disorders. Consult with your insurance company for more information.

Behavioral Sleep Center currently accepts insurance for Cigna, Lyra Health EAP (@ Google only), and Santa Clara County Independent Practitioner’s Association (SCCIPA).

Insurance, Out-of-Network:

Behavioral Sleep Center can provide medical invoices (or super bills) for the following insurance plans and benefits:

  • Anthem Blue Cross, Medicare (starting January 2024), and more.

  • Health Savings Accounts (HSA) and other Flex Spending benefits.

Good Faith Estimate Notice:

You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.

You can ask for a Good Faith Estimate before you schedule a service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

Policies

Confidentiality:  The information disclosed is generally confidential and will not be released to any third party without written authorization, except where required or permitted by law. Exceptions to confidentiality, include, but are not limited to:

  • Reporting child, elder and dependent adult abuse

  • A serious threat of violence towards a reasonably identifiable victim

  • A client who is dangerous to him/herself or the person or property of another.

Cancellation Policy:  Clients are responsible for payment of the agreed upon fee for any missed session(s). Clients are also responsible for payment of the agreed upon fee for any session(s) for which there is a failure to give at least 24 hours notice of cancellation. Cancellation notice should be left on voice mail at my phone number, (408) 780-2248.

Payment Issues:  Clients are expected to pay for services at the time services are rendered.  Cash, checks, and credit cards are accepted.  Sliding scale is available.

Risks and Benefits of Therapy:   Psychotherapy is a process in which the client and therapist discusses a myriad of issues, events, experiences and memories for the purpose of creating positive change to experience life more fully. Participation in therapy may yield benefits such as symptom reduction, improved functioning in day to day functioning, or personal growth and self-awareness.  Participation may also involve discomfort such as remembering and discussing unpleasant events, feelings, and experiences, and require substantial effort to actively participate by remaining honest and willing to change feelings, thoughts, and behaviors. 

Participation in the treatment of sleep may involve temporary, increased sleepiness and require a client agreement with the therapist to follow sleep safety guidelines such as, but not limited to, safety naps, avoidance of operating heavy machinery, etc.

The psychotherapy process provides an opportunity to improve problems or difficulties you may be experiencing, and more deeply understand yourself. Psychotherapy is a joint effort between client and therapist. Progress and success may vary depending upon the particular problems or issues being addressed, as well as many other factors.
